SPA PARTY AGREEMENT

1. Half price deposit is due upon receipt of party packet.
2. Balance is due at time of service.
3. A 48 hour cancellation notice is required for a deposit refund.
4. If there are any no shows for guests or anyone passes on a service the price stated in this agreement stands. FULL PAYMENT of the balance due will be excepted.
5. Modern Image and employees are not responsible for any lost or stolen items.
6. Modern Image and employees are not responsible for any damage.
